Special Needs Intake Form

More Than Conquerors is the adult special needs ministry of LWRBC. All those participating in More Than Conquerors must first register to participate. This ministry is for special needs adults 18 years and older. While primarily for those who attend LWRBC, everyone in the community is invited to register.

The following questions allow us to provide the best experience and safest environment for all our friends within the ministry. Our church leaders and ministry volunteers will respect your family’s right to privacy. Any information shared is communicated directly with those caring for your family member and only on a “need to know” basis. This ministry requires you, as Parents / caregivers to remain on campus. One of the goals of More Than Conquerors is to enable you to participate more fully in the life of our congregation. Staying on campus also gives us access to you if there are any problems that arise that require your assistance. By submitting this application, the undersigned acknowledges this expectation and will remain on site on Sunday mornings.
